Go to script.google.com → New project → paste the Apps Script below → click Deploy → New deployment → Web App, set "Who has access" to Anyone → copy the Web App URL.

Create a Google Sheet. In row 1, add these headers: Timestamp | Slot | Name | Email | Company | Status. Copy the Sheet ID from its URL (the long string between /d/ and /edit).

In the Apps Script, replace YOUR_SHEET_ID_HERE with your Sheet ID. Redeploy, then paste the Web App URL below and click Connect.
